CHANGE IDENTIFICATION (CURRENT AND FUTURE STATE)

This worksheet is designed to identify potentially different perspectives of the proposed change during your assessment phase.
As a small group of change leaders, or individually, try exploring your change from different angles to see what you discover about your assumptions and perspectives. This reflective activity has been designed to showcase two perspectives of the same problem: one that deals with the current state looking forward to the future, and one that takes a future vision and traces the steps back.
An effective change management team should force itself to examine all perspectives to ensure it truly ties into the organization’s goal. Fill out the worksheet in the spaces provided below either individually or as a group and reflect on how your perceptions of your proposed change have developed through the process.
