Annual Safety Survey

October 2017

The annual Safety Survey is now available in the Safety & Infomration Reporting System (SIRS) located in e-Services. The survey for this year is somewhat different from previous years. To make this easier please follow the suggestions and guidelines as outlined here. Attached to the end of The Safety Chain I have created a worksheet you can use for completing your annual CAP safety survey.


First, take your time.  Discuss these questions at a monthly meeting.  Remember, this is for events and so forth which occurred in FY 17. Please review your answers with your unit commander before submitting the survey. Before logging into SIRS, record your replies into the attached worksheet. Some items have two or three parts which require responses. Those items are color enhanced for easy identification. Please respond to all parts of each item.
Once you are satisfied with your responses, copy & paste your responses into the on-line survey. It is recommended you complete entering your responses into the survey in a single session, so please set aside enough time for this activity. There is no provision which permits you to start the survey one day and complete it another day. There is no way to save your responses, so if for some reason you log out before submitting this survey your entries will not be saved and you will need to re-enter all previous entries.
To give me sufficient time to assist you should you need help, please complete the annual CAP Annual Safety Survey no later than 01 Dec 17. This gives you six weeks which should be ample time for safety officers and unit commanders to complete and submit the survey.
I cannot stress how important this survey is. The safety survey helps us to identify how we can help you.
