TURP

Keep this checklist in the TURP folder for the entirety of patient stay. Nursing should use this as a worksheet to make sure all steps of TURP education are complete prior to discharge.

		DO Day 1- DOCUMENT Patient Ed–Add title Procedure/Treatment (#637). Choose all applicable points (don’t need the pre-procedure routine point). Method- explanation. Response- Verbalized Understanding. Add Comment “Continuous Bladder Irrigation”. DOCUMENT Care Plan–Apply template “Nursing-Impaired Urinary Elimination”. Add URINARY ELIMINATION Goal and appropriate interventions. DOCUMENT Doc Flowsheets– Add “CBI” LDA to I&O Flowsheet if not already done in PACU. Complete 3 documentation questions (Infusing, Rate & Drainage Color). UTILIZE Intake/Output Record for TURPS– Clip cheat sheet to front of TURP folder (somewhere visible) for figuring I&O totals.
		DO Day 2- Give patient CareNotes “Transurethral Prostatectomy Inpatient Care”. Review via teach back method. DOCUMENT Patient Ed– Choose appropriate points to Procedure/Treatment (#637). Method- handout & explanation. Response- Teach Back (or other response as applies). Add comment “TURP CareNotes” handout.
		Do Day of Discharge- Pull in .octurpdc to AVS. Review with pt. DOCUMENT Patient Ed– Add Discharge title (#642) and document as appropriate.
