CONCIERGE/MANAGEMENT UNIT ACCESS AUTHROIZATION

This is to authorize and request the Concierge to grant access to the above referenced unit in the Meridian on Sand Key Condominium to the person(s) named above. In giving this authorization and request, the undersigned acknowledges and agrees:


  1. The Concierge will unlock the Unit for the above named person(s) only.

  2. Access must be during Concierge hours. The Concierge is not permitted to leave a Unit unlocked overnight.

  3. Concierge will give access only as authorized. They are not responsible to stay and supervise/oversee/approve any work being accomplished.

  4. Should the undersigned require access be provided to a person who is not specifically named on this authorization, that request must be made directly to the Concierge by the Homeowner or Lessee of the unit via telephone or in writing, providing the name of the person access is requested for and the date that access is to be given. Such access authorizations are considered “one time only” authorizations, and the Homeowner or Lessee must contact the Concierge in advance each time Unit access is required by an individual not named of this form.

  5. Should an individual not named on this authorization request unit access, the Concierge will attempt to contact the Homeowner or Lessee. If the Concierge is unable to reach the Homeowner or Lessee, the individual requesting access will be turned away irrespective of whether that person had been provided access in the past.

  6. The undersigned understands that he/she is fully responsible to the Association for the actions of this guest while on the Condominium Property.

  7. Access is permitted by Concierge/Management staff in case of emergency to inspect for damage (as stated in the condominium documents).


Note: The undersigned agrees to notify the Concierge, in writing, of the termination of this authorization. The Concierge is entitled to assume that this authorization is in full force and effect until a written notice of such termination is received.
Signature of Owner/Lessee

This Concierge Unit Access Authorization automatically terminates when: (1) Homeowner sells his/her Unit at Meridian on Sand Key; or 2) Occupant, if other than Homeowner, permanently terminates his/her residency at Meridian on Sand Key.
