Robotik pelvik-paraaortik lenfadenektomi

Robotik cerrahi, minimal invazif cerrahide günümüzde giderek artan sıklıkta kullanılan bir
yöntemdir. Özellikle jinekolojik onkolojik ameliyatlarda laparoskopik cerrahinin kısıtlılıklarını
yaşamadan operasyon yapabilme seçeneğini sunması oldukça önemli bir avantajıdır. Bu
video sunumunda robotik evreleme yapılan bir endometrium kanseri vakasının sunulması
amaçlanmıştır.
Hasta, 66 yaşında postmenopozal kanama şikayeti ile kliniğimize başvurdu. Yapılan muayene
sonucunda uterin kavite içerisinde 53x52 mm çapında kitlesel lezyon izlenmesi üzerine yapılan
probe küretaj sonucu endometroid tip adenokarsinom, nükleer grade 3, figo grade 3 olarak
geldi. Hastanın onamı alındıktan sonra robotik cerrahi planlandı. Operasyonda Da Vinci Si
kullanıldı. Histerektomi materyali patolojiye gönderildi. Frozen section sonucu tümör çapı 6 cm,
1/2’den fazla myometrial invazyon, grade 3 gelmesi nedeniyle hastaya
bilateral pelvik ve renal
ven seviyesine kadar lenfadenektomi yapıldı. İntraoperatif komplikasyon olmadı. Operasyon
süresi toplam 300 dakika olarak hesaplandı. Çıkarılan lenf nodu sayısı sağ pelvik 19, sol pelvik 18,
paraaortik 39 olmak üzere toplam 76 olarak bulundu. Hasta post-op erken dönemde herhangi bir
komplikasyon yaşamadan 2. günde taburcu edildi.
Robotik cerrahi, özellikle endometrium kanseri evrelendirmesinde tercih edilebilecek önemli bir
minimal invazif yaklaşımdır.

OBJECTIVE: To demonstrate step-by-step extraperitoneal para-aortic lymphadenectomy
METHOD: Extraperitoneal para-aortic lymphadenectomy not only gives a valuable information in
assessing the lymph nodes also, it has therapeutic effect. The Extraperitoneal lymphadenectomy
has less morbidity not only postoperatively even if the patient
required radiation therapy,
compared to retroperitoneal lymph node dissection. In this video we will demonstrate the
robotic extraperitoneal lymphadenectomy in locally advanced cervical cancer
CONCLUSION: Robotic extraperitoneal lymphadenectomy is feasible procedure with less
morbidity

Percutaneous Laparoscopic Total Hysterectomy

STUDY-OBJECTIVE: To demonstrate a percutaneous laparoscopic total hysterectomy
Design: This study is designed to show step-by-step with narration percutaneous laparoscopic
total hysterectomy
Setting: In the last decade there has been wide diffusion of laparoscopic surgery and rapid growth
in awareness of the minimally invasive approach in gynecological surgery (1). Percutaneous
Laparoscopic surgeries have less pain and cosmetic appearance which the patient desires.
Interventions: A 49-year-old lady presented with heavy menstrual bleeding since 2 months.
Pelvic examination showed a bulky uterus measuring about 12 weeks.
Pelvic ultrasonography
showed a 10-cm sized subserous myoma. Hysterectomy with bilateral salpingectomy was
recommended. Patient had percutaneous total hysterectomy with bilateral salpingectomy as
explained somewhere else. For uterine manipulator we used Rumi II. A 5-mm trocar inserted
in the umbilicus and used for Energy instrument, we used Thunderbeat®. Then 2 percutaneous
instrument, Percuvance™ Percutaneous Surgical System (PSS) (The Percuvan-ce™ System, Teleflex
Inc., USA), inserted in the lower bilateral sides of the abdomen. A 3-mm
trocar was inserted in the
upper Right side for a 3-mm camera. The vaginal cuff closed by 2/0 V-Lok. The uterus weighed
about 365 grams. The uterus extracted from the vagina by vaginal morcellation using scalpel.
Total blood loss was 50 ml. The period of the surgery was 140 minutes. The procedure went
uneventful. Patient was discharged on 1st postoperative day.
CONCLUSION: Percutaneous laparoscopic total hysterectomy is a feasible procedure. Patient will
experience less pain and will have a cosmetic appearance of the abdomen.
