Total laparoskopik histerektomi sonrası dren kullanımı

Son yıllarda jinekolojide laparoskopi sıkça tercih edilen bir yöntem halini almıştır. Deneyimler
eskiye oranla giderek artmaktadır. Ancak dren kullanımı konusunda cerrahlar arasında fikir
ayrılıkları halen devam etmektedir. Laparoskopik histerektomi sonrası komplikasyon gelişmeyen
ve sonrasında komplikasyon beklenmeyen hastalarda rutin dren kullanılması önerilmemektedir.
Ancak kanama şüphesi bulunan vakalarda tercih edilebilmektedir. Bu araştırmada amacımız
benign jinekolojik nedenlerle laparoskopik histerektomi yapılan hastalarda dren kullanımının
postoperatif avantaj ve dezavantajlarını karşılaştırmaktır.
MATERYAL-METOD: Dr.Lütfi Kırdar Kartal Eğitim ve Araştırma hastanesinde 2015 ile 2017
yılları arasında yapılan total laparoskopik histerektomiler retrospektif olarak incelenmiştir.
Hastalar histerektomi sonrası dren konulmasına göre 2 gruba ayrılmıştır. Grup 1: dren konulan
grup, grup 2; dren konulmayan gruptur. İstatistiksel değerlendirmeler SPSS 17.0 programıyla
gerçekleştirilmiştir. Kontinü
veriler student T test ile, kategorisel veriler ise ki-kare testi ile
karşılaştırılmıştır.
BULGULAR: Histerektomi yapılan 64 hastadan grup 1’de 35 hasta grup 2’de 29 hasta yer almıştır.
Grupların demografik, laboratuar ve klinik parametrelerle ilgili verileri tablo 1’de gösterilmektedir.
Operasyon süresinin grup 2’de daha fazla olduğu ve istatistiksel olarak sınırda anlamlı olduğu
belirlenmiştir. Özellikle grup 1’de preoperatif ve postoperatif
hemoglobin ve hematokrit
farklılıklarının istatistiksel olarak anlamlı şekilde daha fazla olduğu görülmektedir. Grup 1 ve grup
2’deki hemoglobin düşüşü sırasıyla 1.6±0.9 ve 1.1±0.6 (p=0.017) iken hematokrit düşüşü 4.8±2.7
ve 3.3±2.2 (p=0.014) olarak belirlenmiştir.
SONUÇ: Benign jinekoloji nedenlerle total laparoskopik histerektomi yapılan hastalarda dren
kullanılması hemoglobin ve hematokrit değerlerinde düşüşe neden olabilir. Bu nedenle bu grup
hastada dren kullanılmasını gerekli görülmedikçe önermiyoruz.

Four ports access versus three ports access total laparoscopic hysterectomy

BACKGROUND/AIMS: TLH (Total laparoscopic hysterectomy) is commonly done with three ports
consisting of an optical port and two accessory ports. Recently, single port usage is increasing.
My aim is to discuss pros and cons of using the fourth port in addition to three ports.
METHODS: Datas of patients who underwent TLH for a benign cause between 01.07.2015 and
10.13.2017 in Dr. Lütfi Kırdar Kartal Training and Research Hospital were obtained, retrospectively.
Patients who re-operated or underwent laparotomy converted from TLH
were not included in the
study. Sixty four patients were included in the study. Preoperative and postoperative data were
obtained from medical records. Patients were divided into two groups: group 1 was operated
with 3 ports and group 2 was operated with 4 ports. Two groups were compared in terms of
demographic, laboratory and clinical parameters. SPSS 17 was used for comparisons. Student
t-test was used for continuous data and chi-square test was used for categorical data. P< 0.05 was
considered statistically significant.
RESULTS: Sixty four TLH cases were included in the study. There are 29 patients in group 1 and 35
patients in group 2. The
data related to demographic, laboratory and clinical parameters of the
groups are shown in table 1. When table 1 is examined, there is no difference between groups
in terms of demographic characteristics and blood parameters.
Clinically, there is 27.6% fever
(≥37.5C) in group 1 and 5.7% fever in group 2 (p=0.034). When group 1 patients are examined, it
is seen that the rate of drain usage was 93.1% whereas in group 2 this ratio is 22.9% (p< 0.0001).
CONCLUSION: In TLH, adding a port to classic 3 ports does not affect operation time, postoperative
blood value and discharge time. I think that fever is associated with drain. Surgeons should not
avoid to use additional ports if necessary.
